1.    海洋性貧血之分子診斷
SEA type是國內甲型海洋性貧血最常見的基因型(αα/--SEA),其第十六對染色體中的一條發生20kb的刪除,這刪除包括兩個α-globin基因。本實驗室以E. coRⅠ作用的genomic DNA為檢體,並取KHz-globin基因的部分序列(由柯滄銘醫師提供),在含有Digoxigenin-dUTP之dNTP條件下,以Nick translation的方式所製成的探針,進行Non-radioactive Southern blot。正常人的檢體,探針會和4.5 Kb和23 Kb的DNA片斷進行雜合反應,在SEA type carrier的檢體則可得4.5 Kb、16Kb和 23Kb三個不同的DNA片斷,另外在homozygous SEA type(--SEA/--SEA)的胎兒,則可得4.5 Kb和16 Kb大小的DNA片斷。一般而言,SEA type可用PCR進行快速診斷,但為避免PCR在診斷上的誤差,部份檢體需以Southern blot作進一步的確認。但為了更快速有效地進行產前診斷同合子型的胎兒,吾人設計了使用即時定量PCR的方法供長庚醫院檢驗醫學部分子診斷實驗室進行快速準確的產前分子診斷用。未來將對乙型海洋性貧血研發可能的基因治療而努力。

2.    腫瘤特異血漿DNA在攝護腺癌之臨床意義
在大多數遺傳性癌症患者的早期診斷往往是由於特定的基因發生某些變異,而這種情形也出現在一些攝護腺癌病人中。由此看來遺傳基因亦在攝護腺癌的發生扮演重要的角色。根據近年來的報告,在英、美等國攝護腺癌的發生居高不下,而行政院衛生署最近一份報告指出,攝護腺癌已躍升國人男性十大癌症之第5位,由此可見此種惡性腫瘤之嚴重,令吾人不敢忽視其威脅。隨著分子生物技術進步,一些可能和攝護腺癌有關聯的基因如GSTP1,Drg-1等亦陸續被提出,但尚無決定性結論。而癌細胞亦因其本身特性較正常細胞更易凋亡而釋出DNA於血液循環中。本研究室與林口長庚醫院泌尿科合作,針對數個可能與攝護腺癌的發生有關的基因變異如PSMA,GSTP1,及Drg-1, AR等設計分子檢驗方法(傳統聚合脢連鎖反應、限制酵素作用、及甲基化敏感性即時定量反轉錄聚合脢連鎖反應)以發展臨床試驗提供臨床應用。來對我國攝護腺癌病人作大規模研究,以判斷其是否具有臨床診斷應用價值或作為預後指標,並以此為出發點,且使用微陣列分析方法系統性尋找和攝護腺癌發生或轉移有關的基因指標,期更深入探究引發攝護腺癌的遺傳基因確切位置及功能。

3.    遺傳疾病之分子診斷

我們與台北、林口、嘉義長庚醫院婦產科及小兒科醫師合作,針對一些遺傳疾病的基因變異研究分子診斷方法以快速鑑定出基因變異和表現型間的關係。
